Reviewing a Medical Record

You may review your or your children's protected health information in the health information management department at Memorial Hospital during business hours. We request you call 24 hours in advance to ensure the records are available upon your arrival.

Obtaining a Copy of a Medical Record

You may also request a copy of your medical records. However, Memorial Hospital is required by law to obtain your written permission before releasing any copies. If you desire to receive a copy of your medical records, please print, complete and deliver an Authorization to Disclose Protected Health Information form to one of our locations. We are not able to accept or respond to email requests.
