Registration

The registration/admissions process for Memorial Hospital for Children patients goes through our Patient Access department. Patient Access will welcome you to Memorial and begin your paperwork. We are available to process your registration in convenient locations throughout our medical facilities, including Memorial Hospital Central, Printers Park Medical Plaza, Briargate Medical Plaza, HealthLink and Springs Medical Center.

Here’s what you can expect:

  • Sign forms that authorize release of information to your insurance provider, assign insurance benefits to the hospital and explain our privacy policies
  • Provide photo identification
  • Provide your health insurance card at each visit to ensure we have your most current information and to verify authorizations (we’ll photocopy your card)
  • Pay any co-pays and/or deposits
  • Consult with us on financial counseling/programs for assistance with bill payment if needed

Providing us with accurate and complete information during the registration process is vital in ensuring your hospital bill will be processed correctly and quickly.
